When active he is a full-time, every-snap QB1 in Andy Reid's offense with no committee element — the only competition for the job is whatever veteran backup is behind him, who plays only if Mahomes is held out or shut down. The live variable is not usage but whether the knee changes how much he scrambles and extends plays, which is where a meaningful chunk of his rushing production and his off-schedule throwing has historically come from.
He tore the left (non-plant, for a right-handed thrower) ACL along with the LCL on Dec. 14, 2025, had surgery the next day with Dr. Dan Cooper, and was throwing again by March. He said in late July he felt "amazing" and was "extremely confident" about Week 1. Quarterbacks are the position that rehabs ACLs best because the job is not built on cutting, and Reid's system protects him with quick game and structure. If the knee holds up, he is the same passer inside the pocket he always was.
It was a two-ligament repair, not a clean isolated ACL, and Mahomes himself said the knee will likely bother him for the rest of his career. He did not play in the preseason — the first time in his career — and Reid stayed pointedly non-committal about clearance. Post-ACL quarterbacks routinely lose the scramble and the extend-the-play magic in year one, and the recurring "questionable" tag suggests the team is managing him week to week rather than treating him as fully past it.

Whether the knee costs him his off-script rushing and improvisation — the part of his game that separates him — or only limits him early before he trusts the leg. Analysts broadly agree his arm and pocket play are intact; they split on the mobility, and on how much the week-to-week designation reflects genuine risk of him being held out versus routine maintenance on a rebuilt knee.
